SAN FRANCISCO - Federal investigators say they are focusing on human error as the cause of a light-rail train crash in San Francisco that injured dozens of people after finding no indication of mechanical or systems failures.

The National Transportation Safety Board said Wednesday the operator of the San Francisco Municipal Railway train in Saturday's collision told investigators he blacked out before the crash.
The NTSB says it found no problems with the train's brakes or any other mechanical issues that would have led to the accident. Investigators still are reviewing the health history of 61-year-old Henry Gray, the train operator.
Gray, a 30-year transit agency employee, switched from driving buses to light rail trains in 2007. State records show he has a clean driving record.
